Notice of Privacy Practices

Taylor Made Integrative Therapy is committed to protecting your personal and health information. This policy explains how we collect, use, and safeguard that information, consistent with HIPAA.

Information we collect

We collect the personal and health information you provide — such as your contact details, health history, and the findings from your assessments and treatment — for the purpose of providing your care.

How we use and share it

  • For your treatment — we may share relevant information with other providers involved in your care, such as a referring physician or imaging center.
  • For payment — we are a cash-based practice and do not bill insurance. If you request a receipt or superbill for HSA/FSA or personal reimbursement, we will prepare it using your information.
  • For our operations — we may use your information for scheduling, appointment reminders, quality improvement, and staff training.
  • When required by law — we will disclose information when the law requires it, such as a court order or mandatory reporting.
  • With trusted vendors — we may share information with service providers (such as our scheduling and records systems) who are contractually required to protect it.

We do not sell your information, and we will not use it for marketing without your written permission, which you may revoke at any time.

How we protect it

Your records are kept in secure, access-controlled systems, and only staff who need your information to do their jobs can access it.

Your rights

You may review and request a copy of your records, request corrections, ask how your information has been shared, request restrictions on its use, and receive a copy of this policy at any time.
